"Add Unclaimed" form pre-fills stale confirmation number from bookings filter

When a user has a confirmation number typed into the bookings search filter on the Supplier Statements page and then clicks "Add Unclaimed," the form pre-fills with that filter value instead of opening blank. This means if they're adding multiple unclaimed bookings in a row, the form keeps carrying over the previous confirmation number rather than clearing for a fresh entry.
The user has to manually clear the bookings filter before clicking "Add Unclaimed" each time, which is error-prone and has already caused incorrect data entry.
The "Add Unclaimed" form should always open with a blank confirmation number field, regardless of what's in the bookings search filter.
